Important

Requests for prescriptions cannot be taken over the telephone.

  • An online account.
  • The name of the medication and dosage.
  • To be at least 17-years old.

If you have one of the following medical conditions you may be entitled to prescription charge exemption.  Please ask at Reception for a Form FP92A.

  • A permanent fistula
  • Epilepsy for which continuous anti-convulsive therapy is required
  • Diabetes mellitus and treatment is not just by diet alone
  • Myxoedema (hypothyroidism requiring thyroid hormone replacement)

Alternatively, you can get a prescription:

  • In person – Tick the items you need on the right-hand section of your previous prescription form and leave your prescription request at Reception
  • By email – send your request to d-ccg.topshamscripts@nhs.net
  • By post – Tick the items you need on the right-hand section of your previous prescription form and send your prescription request to us. If you prefer, we can post it back to you (you will need to provide a stamped, addressed envelope), or can send it to a local pharmacy for you to collect it from there.
Further information on repeat prescriptions

Repeat prescriptions take 72 hours for the surgery to process, sign and send to your Pharmacy.

Your Pharmacy will then require on average 3-5 working days in addition to this to dispense the prescription and have it ready for collection. Each Pharmacy’s processing time varies so please check with the chemist you use, how long they need to dispense prescriptions.

Please do not request your prescription too early, apart from when the Surgery is closed on a Bank Holiday or the Christmas period, as the computer will not allow the request to be processed.

We are keen to ensure that patients with ongoing medical problems are monitored regularly.  If the date for your next review with a Doctor or Nurse has passed you will be asked to make an appointment.

Nominating a pharmacy

When you order a repeat prescription online, you will have to nominate a pharmacy to prepare your prescription and for you to pick it up from.

You can nominate a pharmacy by using the NHS App or logging into the NHS website using a web browser. A high street pharmacy is where you collect your prescriptions in person. If you have chosen an online-only pharmacy, then they will send your prescription in the post.

If you order your prescription online:

  • You will no longer need to collect paper prescriptions from our surgery.
  • Your nominated pharmacy will have your prescriptions ready to collect until you change or remove them.

For more information about nominating a pharmacy, visit the Nominate a Pharmacy page on the NHS website.

Menopause prescriptions

From the 1 April there is a new scheme available enabling access to a year’s worth of menopause prescription items for the cost of two single prescription charges. A pre payment certificate (PPC) can be applied for through NHS Business Services Authority or in person at a Pharmacy registered to sell PPCs.

How your pharmacy can help

Pharmacists are qualified healthcare professionals who can help you with minor health concerns. They can offer medical advice and over-the-counter medicines for minor illnesses like coughs, colds, tummy problems and aches and pains.

For symptoms that are more serious, we suggest visiting your GP or nurse. You can book an appointment by going to our Appointments page.

Prescription charges

The NHS prescription charges apply to England only. The Government provides help for those who have difficulty paying NHS prescriptions. You can find out more about whether you’re entitled to support from the Government on the NHS Prescription charges page on the NHS website.

More information and advice about NHS England prescription charges are on the Who can get a free prescription page on the NHS website.
